The pharmacist will work with the interdisciplinary team on drug information, dosing adjustments, anticoagulation monitoring, TPN, and stewardship to ensure optimal patient outcomes. Nights shift is included.

Qualifikationen

• Pharmacist must have an accredited Doctor of Pharmacy program completion.
• Current Florida Pharmacist License required.
• Clinical pharmacist experience or training preferred.

Aufgaben

• Provide drug information to patients and healthcare teams including medical, nursing, and pharmacy staff.
• Supervise and delegate tasks to technicians and support staff to ensure timely prescription verification, preparation, and distribution.
• Provide clinical pharmacy services (renal/hepatic dosing, pharmacokinetic dosing, anticoagulation monitoring, TPN, antimicrobial stewardship).
• Review and interpret medication orders, labs, and clinical data to ensure safe, optimal therapy and document interventions.
• Collaborate with interdisciplinary team to manage drug regimens and support patient-centered care.
