Job summary

Hartford HealthCare in the Greater Manchester Region seeks a Pharmacist to manage medication control, monitoring and dispensing for patients of all ages in accordance with department procedures. You will supervise pharmacy technicians and advise nursing staff on safe medication administration.

Responsibilities include optimizing drug therapy, providing drug information, participating in health care team activities, ensuring timely, cost-effective therapy, and occasionally serving as charge

Qualifications

• PharmD degree required.
• Current CT Pharmacist license or eligible within 45 days of hire.
• PGY-1 Pharmacy residency preferred.
• Experience with EPIC systems preferred.

Responsibilities

• Optimizes drug therapy for the general patient population by designing drug therapy that is appropriate, correct, and cost-effective to maximize positive outcomes.
• May provide direct patient care for pain management via evaluation, recommendations and follow-ups as requested by physicians or on the critical path.
• Maintains safety of drug therapy by using screening mechanisms and input from health care providers, patients and families.
• Provides drug information through participation on Health Care Team activities and as a liaison between the HCT and the pharmacy.
• Investigates and resolves drug/pharmacy problems to ensure high-quality medication use.
• Documents interventions and outcomes to support quality improvement.
• Demonstrates competencies to meet age-specific patient needs.
• Periodically may function as charge pharmacist.
