Job summary

Winslow Indian Health Care Center is seeking an experienced Clinical Pharmacist to provide direct pharmaceutical care in the outpatient program. The role includes managing selected chronic disease states, counseling patients, and collaborating with the medical staff within WIHCC guidelines.

The position requires a PharmD and state licensure, BLS and ACLS within a year. Navajo language skills preferred; a valid driver’s license is required.

Qualifications

• Doctor of Pharmacy degree required and licensed by a State Board of Pharmacy
• BLS certification required
• ACLS certification required within one year of hire
• Bilingual English and Navajo language preferred
• Must maintain a valid unrestricted driver’s license
• Background check and suitability for Child Care position

Responsibilities

• Provide direct pharmaceutical care in outpatient pharmacy program
• Monitor and adjust therapy for chronic disease states and acute patients
• Collaborate with physicians and other providers to develop protocols
• Document clinical activities and update medication allergies in records
• Counsel patients on medications, storage, side effects and adherence
• Initiate prescriptions for formulary agents and renew as needed
