Job summary

JobRx, Inc. is seeking a Clinical Pharmacist to deliver exceptional pharmaceutical care within a respected medical center. You will work closely with physicians and nursing staff to ensure safe, effective medication management for patients.

The role involves patient-specific medication therapy planning, drug information education for staff and families, and ongoing monitoring of therapy with dose adjustments as needed. A Pharm.D.

Qualifications

• Pharm.D. degree preferred; licensed pharmacist required.
• Active pharmacist license in the respective state.
• Minimum 2 years of clinical hospital pharmacy experience preferred.

Responsibilities

• Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient-specific medication therapy plans.
• Conduct medication reviews, assess drug interactions, and recommend interventions to optimize therapy.
• Provide drug information and education to healthcare professionals, patients, and families.
• Monitor patient responses to medications, adjust doses, and provide guidance on side effects or adverse reactions.
• Participate in medication reconciliation processes and contribute to safety initiatives.
• Contribute to development and implementation of evidence-based pharmacy protocols and guidelines.
• Ensure compliance with regulatory standards, policies, and procedures related to medication management.
• Contribute to training and mentoring of pharmacy students and residents.
