Job summary

Lifepoint Health® is seeking a Pharmacist to coordinate and provide pharmaceutical care for patients, collaborating with an interprofessional team to optimize safe, effective medication use.

The role involves reviewing orders, educating staff and patients, and ensuring compliance with quality and regulatory standards. It includes outpatient anticoagulation clinic responsibilities and ongoing education in a non-exempt capacity.

Qualifications

• Doctor of Pharmacy degree required and valid Kentucky Pharmacist license.
• BCLS required; ACLS preferred.
• Excellent English communication and ability to document and educate.
• Experience with reviewing orders, drug information, and software systems.

Responsibilities

• Coordinate and provide pharmaceutical care with interprofessional team.
• Review, interpret, and evaluate medication orders for safety and efficacy.
• Educate staff, patients, and students on drug therapies.
• Document interventions and outcomes for quality improvement.
• Verify accuracy of orders and supervise technicians in preparation of meds.
• Manage outpatient anticoagulation clinic activities, including dosing and scheduling.
• Participate in performance improvement and continuing education.
• Perform additional duties as assigned.
