Qualifications

• PharmD degree from an accredited institution.
• Minimum of 5 years of clinical pharmacy experience.
• BCPS or equivalent certification preferred.
• Experience with pharmacokinetics dosing, renal dosing, anticoagulation and medication reconciliation.
• Excellent communication and teamwork skills.
• Knowledge of pharmacy laws and regulations.
• Proficient in EHR and pharmacy information systems.

Responsibilities

• Provide pharmacokinetics dosing support and renal dosing protocols.
• Manage anticoagulation protocols for patient safety and efficacy.
• Conduct medication reconciliation during transitions of care.
• Support transitions of care with other healthcare professionals.
• Review drug interactions and allergies to prevent ADEs.
• Support antimicrobial stewardship to optimize antimicrobial use.
• Provide clinical pharmacy services including patient counseling.
• Prepare sterile compounds per procedures and guidelines.
• Verify medication orders for accuracy and appropriateness.
• Participate in multidisciplinary committees to develop policies.
• Stay updated with advancements in pharmacotherapy and apply them.
