Qualifications

• Active, unrestricted CA pharmacist license.
• Doctor of Pharmacy degree required.
• Experience with compounding infused drugs.
• Proven record evaluating and monitoring drug therapies for home care patients.
• Excellent verbal and written communication skills.

Responsabilités

• Accept and dispense prescription medications.
• Monitor patient medication compliance and over-usage patterns.
• Ensure medicines are within legal and safety standards.
• Advise patients on medicines and potential reactions.
• Provide consultation on prescription and non-prescription meds, dosing and interactions.
• Educate patients on handling medical devices and self-monitoring.
• Supervise pharmacy staff and manage inventory.
