Job summary

Covenant Health is seeking a Clinical Pharmacist to provide medication distribution and drug information throughout the hospital and to the primary care team under the direction of the Director of Pharmacy.

The role includes monitoring therapy, performing chart reviews, counseling patients discharging from the institution, and collaborating with physicians, nurses, and other staff across departments to optimize patient safety and outcomes.

Qualifications

• Current New Hampshire license to practice Pharmacy required.
• Pharm.D. degree preferred.
• Board Certification in Pharmacotherapy (BCPS) preferred.
• 1–3 years of clinical pharmacy experience preferred.
• Residency training or equivalent patient care experience preferred.

Responsibilities

• Provide safe and efficient medication therapy to patients.
• Monitor medication orders for dosing, interactions, and contraindications.
• Conduct daily clinical chart reviews and intervene as needed.
• Offer drug information and cost-effective treatment recommendations to providers.
