We envision a world where the medication journey is humanized for patients at every step, and we are seeking a highly skilled and dedicated Ambulatory Care Clinical Pharmacist to join our team.

About The Job

As a Clinical Pharmacist, you will be responsible for providing comprehensive medication management services to patients in an ambulatory care setting. Your expertise in medication therapy management and patient counseling will play a critical role in optimizing patient outcomes in collaboration with prescribers. You will support a variety of clinical operations and quality measures related to medication management.

What You Will Do
  1. Provide medication reconciliation services contributing to safe Transitions of Care for patients.
  2. Close clinical gaps to improve CMS Star ratings and HEDIS measures.
  3. Conduct comprehensive medication reviews to assess appropriateness, efficacy, and safety of drug therapy for individual patients.
  4. Maintain accurate and up-to-date patient records, including medication profiles.
  5. Document clinical assessments, recommendations, and follow-up plans in accordance with regulatory and organizational requirements.
  6. Identify and resolve potential and/or actual medication related problems by formulating evidence-based recommendations for medication therapy changes when necessary.
  7. Serve as a trusted advisor to patients and prescribers answering medication related questions, addressing concerns, and providing outstanding customer service.
  8. Collaborate closely with other healthcare professionals, including physicians, nurses, and other allied healthcare staff, to ensure coordinated patient care.
  9. Participate in quality improvement initiatives to enhance the delivery of comprehensive medication management services.
  10. Stay current with the latest developments in pharmacotherapy, clinical guidelines, and best practices through ongoing professional development.
  11. Champion diversity, equity, and inclusion efforts, fostering an equitable workplace.
  12. Other duties as assigned.
What You Will Need
  • Doctor of pharmacy (Pharm.D.) from an accredited school of pharmacy with 3 years clinical experience.
  • Current active pharmacist license in Missouri. Licensure in multiple states is preferred.
  • National Provider Identifier (NPI) required.
  • Currently hold a Board of Pharmaceutical Specialty Certification or have completed an ASHP Residency with two years of clinical experience is preferred. (BCACP is preferred.)
  • Spanish or other non-English language fluency is preferred.
What You Will Bring To The Table
  • Experience in the medication reconciliation process.
  • Previous experience in ambulatory care or clinical pharmacy.
  • Excellent communication (verbal and written) and interpersonal skills.
  • Ability to handle a large workload while working efficiently and accurately.
  • Demonstrated success improving health outcomes for assigned patient populations.
  • Commitment to patient-centered care and continuous quality improvement.
  • Extensive knowledge of pharmacy and its related subdisciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug use review, quality assurance, legal, regulatory, and standards of pharmacy practice.
  • Ability to integrate within the healthcare team.
  • Proficient at working with or learning how to work with Electronic Health Records Systems.
  • Works independently to meet expected deadlines and deliver excellent patient care.
  • Understanding of the principles of clinical integration and population health management.
  • Ability to work collaboratively as a member of an interdisciplinary team, establish and maintain effective working relationships with other healthcare professionals, practitioners, patients, and families.

Additional Information

This is a PRN (as needed) position with flexible and variable hours.

Hourly wage: $48-63/hr
