The Clinical Excellence Quality (CEQ) Pharmacy Market team supports Optum’s Care Delivery Organizations by providing patient-centered, clinically collaborative medication consultations. Our focus is on telephonic and digital engagement strategies to close STARs measure gaps, such as medication adherence and care for older adults. Our interdisciplinary team includes Pharmacists, Pharmacy Technicians, Licensed Vocational Nurses, and Care Coordinators.

The Clinical Pharmacist (CP) reports to the Manager of Clinical Pharmacy, Market Support. Responsibilities include telephonic, patient-centered clinical consultations, annual medication reviews, functional assessments, and acting as a field consultant for providers and clinics via in-person and virtual meetings. The CP must meet productivity and quality metrics and work a flexible schedule aligned with provider operational hours. If located in MST or PST, remote work flexibility is available.

Primary Responsibilities
  • Balance telephonic patient outreach with provider-facing activities (virtual and in-person)
  • Develop market business plans to motivate providers and large group leadership to improve STAR measures (e.g., medication adherence, statin use in diabetes and cardiovascular disease)
  • Interpret Stars and HEDIS reports, provide executive summaries, and share best practices and patient barriers
  • Serve as the subject matter expert for STAR activities within the market, collaborating across teams
  • Lead or participate in performance review meetings and present performance summaries
  • Analyze provider and group structures to identify effective strategies
  • Assist in creating training and analytical materials for Stars and HEDIS
  • Identify decision-makers and barriers within provider groups to tailor communication
  • Develop user-friendly initiatives to support practice success
  • Make outbound calls to members to gather information related to HEDIS and quality measures
  • Conduct annual medication reviews emphasizing patient assessment, medication safety, and interactions
  • Assess pharmacotherapy, claims, medical history, and disease states
  • Identify medication adherence barriers and develop plans to improve adherence
  • Provide patient-centered medication consultations and develop follow-up action plans
  • Maintain current knowledge of chronic diseases and pharmacy practices relevant to adult and geriatric populations
  • Perform other duties as assigned
Qualifications
  • Doctor of Pharmacy or equivalent experience
  • Active pharmacy license in the primary state and Oregon; willingness to obtain licenses in other states as required
  • 5+ years of clinical pharmacy practice
  • 2+ years of experience with Medicare STARs & HEDIS measures
  • 2+ years of provider-facing interactions and education
  • 2+ years of data analysis and quality reviews
  • Proficiency in Microsoft Office
  • Ability to adapt to a rapidly changing, matrixed environment
  • Strong organizational, planning, and independent working skills
Preferred Qualifications
  • MBA/MPH
  • Postgraduate training (Residency/Fellowship)
  • Board certification by the BPS
  • Certified Geriatric Pharmacist
  • Bilingual skills (Spanish, Mandarin, Vietnamese, Korean preferred)
  • Excellent communication and relationship-building skills

The salary range is $89,800 to $176,700 annually, based on experience, location, and qualifications. Benefits include comprehensive health plans, incentives, stock purchase, and 401k.
