The role emphasizes clinical care, patient profiles, regulatory compliance, and education of staff. A PharmD or Bachelor’s in Pharmacy with licensure and LTC experience is preferred; rotating shifts, weekend work, and on-site duties apply.

Qualifications

• Bachelor’s degree in Pharmacy required; PharmD preferred.
• Strong clinical judgement and patient care focus.
• Excellent written and verbal communication with healthcare teams.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage and interactions.
• Provide clinical recommendations to physicians and staff for therapy changes.
• Maintain up-to-date patient profiles with meds, labs and history.
