Qualifications

• ACPE-accredited PharmD degree.
• Current Pharmacist licensure in practicing state.
• Completion of PGY-1 residency or 3 years clinical pharmacy experience.
• Board Certification preferred.

Responsibilities

• Lead clinical pharmacy services and patient-centered care across assigned services.
• Collaborate with healthcare providers to optimize therapy and ensure safe medication use.
• Provide discharge medication review and counseling; participate in quality initiatives and education.
