Specialty Medical Staffing is seeking an experienced Clinical Home Infusion Pharmacist specializing in Total Parenteral Nutrition (TPN) for an opportunity with our partner facility in Orange, California. In this role, you will lead the clinical oversight, therapy optimization, and management of complex home parenteral nutrition regimens. You will serve as a key clinical expert across sterile compounding standards (USP <797>), individualized TPN dosing, and interdisciplinary patient care.
TPN Therapy Optimization: Evaluate patient lab trends, nutritional parameters, and clinical conditions to calculate, initiate, and adjust customized TPN formulations.
Order Verification & Compounding Oversight: Review prescription orders for clinical rationale, accuracy, and safety while ensuring full compliance with USP <797> cleanroom standards and sterile preparation protocols.
Care Coordination: Collaborate directly with prescribers, registered dietitians, home care nurses, and discharge planners to facilitate smooth hospital-to-home transitions.
Patient & Caregiver Education: Educate patients and families on TPN administration, line care, side-effect management, and emergency response protocols.
Quality & Regulatory Compliance: Maintain precise clinical documentation, participate in performance improvement initiatives, and report medication safety events.
Leadership & On-Call Coverage: Supervise pharmacy technicians and support staff as assigned, and participate in on-call rotations to maintain continuous 24/7 care delivery.
Education: PharmD or B.S. in Pharmacy from an accredited pharmacy school.
Licensure: Licensed in AL, MS and/or MD.
Experience: Minimum of 2 years of home infusion pharmacy experience, with direct expertise managing TPN patients.
Technical Skills: Strong proficiency in aseptic techniques, cleanroom protocols (USP <797>), and home infusion equipment/pumps.
Core Competencies: Strong clinical judgment, effective communication, and the ability to thrive in a fast-paced clinical setting.
