Job summary

Endeavor Health is seeking a Clinical Pharmacist to provide comprehensive pharmaceutical care for oncology patients at Swedish Hospital in Chicago and affiliated sites. Per diem work with rotating shifts and travel to multiple centers is required.

Eligible candidates must hold Illinois Pharmacist licensure and have a PharmD or BS in Pharmacy, with oncology experience preferred. Minimum three years as a clinical pharmacist and willingness to rotate to Kellogg Cancer Center & Infusion centers

Qualifications

• License: Current Illinois Registered Pharmacist.
• Education: BS or PharmD (preferred) degree from an accredited school of pharmacy.
• Certification: Board certification in oncology training or equivalent experience preferred.
• Experience: At least 3 years of experience as a clinical registered pharmacist (excluding externships as a pharmacy student).

Responsibilities

• Provide and document comprehensive pharmaceutical care to NorthShore patients with cancer and other conditions related to cancer.
• Provide and document medication education and symptom management to oncology patients and caregivers.
• Review and process chemotherapy and supportive care orders plans for clinical appropriateness.
• Prepare and dispense anti-cancer and supportive care medications for oncology patients following best practices and the organization's policies and procedures.
• Design or update chemotherapy plans, supportive care regimens and monitoring plans for oncology patients.
• Design effective educational materials for patients on chemotherapy.
• Provide chemotherapy infusion reaction management to oncology patients.

Hourly Pay Range

$58.00 - $89.90 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights

• Position: Clinical Pharmacist
• Location: Swedish Hospital, Chicago and 3060 Salt Creek Ln, Arlington Heights
• Per Diem: As needed
• Required Travel: Yes. Must be willing to rotate to other Endeavor Health Kellogg Cancer Center & Infusion center sites as needed
• Hours: Monday to Sunday 0700 to 1800 depending on the day; rotating weekends and holidays

What you will do

Patient Care

• Provide and document comprehensive pharmaceutical care to NorthShore patients with cancer and other conditions related to cancer
• Provide and document medication education and symptom management to oncology patients and caregivers
• Review and process chemotherapy and supportive care orders plans for clinical appropriateness
• Prepare and dispense anti-cancer and supportive care medications for oncology patients following best practices and the organization's policies and procedures.
• Design or update chemotherapy plans, supportive care regimens and monitoring plans (care plans) for oncology patients
• Design effective educational materials for patients on chemotherapy
• Provide chemotherapy infusion reaction management to oncology patients

Teaching/Education

• Provide consultative services to other health care professionals on anti-cancer drug therapy and monitoring
• Provide medication education to pharmacists and other health care professionals
• Precept learners (PGY2 oncology resident, PGY1 residents, and students) who are on rotation with the team

Performance Improvement

• Review protocols within the EHR for clinical appropriateness based on current guideline recommendations
• Coordinate and/or collaborate with other health care professionals to carry out quality improvement projects and develop institutional best practices
