Description

Summary:


All pharmacists fill orders for drugs, monitor patient drug therapies, and provide drug information. Pharmacists deliver pharmacy services with compassion, excellence, and efficiency.


Responsibilities:


  • Meet expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.

Operational Duties and Responsibilities


  • Prepare and dispense drug orders per physician request according to established policies, procedures, and protocols.
  • Interpret drug orders (verbal and written) and transcribe/verify into computerized patient medication records accurately. Maintain accurate and complete patient drug records.
  • Compound and dispense pharmaceuticals, including non-sterile and sterile products, chemotherapy, and parenteral nutrition products accurately.
  • Issue controlled substances to patient care areas and maintain records as required by law and institutional policies.
  • Provide services efficiently and in a timely manner.
  • Maintain current pharmacist license, ensure CE records and licensure are up-to-date, and attend staff meetings.
  • Complete all competency and skills assessment requirements.
  • Supervise and direct pharmacy support personnel, verifying the daily activities of pharmacy technicians.
  • Ambulatory care Pharmacist: Provide ambulatory pharmaceutical care services utilizing approved protocols or collaborative agreements, including managing patient drug therapy, anticoagulation management, patient education, and overseeing medication preparation and dispensing. Perform duties in clinical areas such as anticoagulation clinics, intensive medical home, transition of care, discharge medication reconciliation, etc.

Clinical Duties and Responsibilities


  • Ensure safe, appropriate, and cost-effective drug therapies for patients according to policies and protocols.
  • Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of drug and dose.
  • Assist with pharmacokinetics consult service and renal drug dosing per hospital protocol.
  • Read, extract, and interpret information in patient medical records accurately.
  • Detect and report suspected adverse drug reactions promptly.
  • Sustain the hospital drug formulary, minimize non-formulary procurements, utilize therapeutic substitution protocols, and promote rational drug therapy selection.
  • Provide clinical consultation and clarification to practitioners as appropriate.
  • Provide accurate and timely drug information to the hospital's professional staff.
  • Educate patients and their families about medications per institutional protocol.
  • Participate in quality improvement and medication use review activities, including data collection, monitoring, inspections, and record maintenance.
  • Conduct designated interventions such as IV to PO conversions, renal dosing, pharmacokinetic support, and anticoagulation, reporting interventions per departmental standards.
  • Participate in antibiotic stewardship, opioid stewardship, and shortage management activities as assigned.
  • Attend interdisciplinary rounds when assigned.
  • Provide services efficiently and in a timely fashion.

Occupational Hazards


  • Potential exposure to hazardous substances, including chemotherapeutic and cytotoxic drugs, and risk of sticks or cuts from needles and sharps.
  • Potential for musculoskeletal injuries if proper lifting techniques are not used.
  • Potential exposure to biologic material during direct patient care activities.

Job Requirements:


Education/Skills


  • Doctor of Pharmacy or BS Pharmacy degree required.

Experience


  • Hospital experience preferred.

Licenses, Registrations, or Certifications


  • Pharmacy registration in the state of practice.
  • Pharmacy licensure within 60 days of start date.
  • BLS, ACLS, and PALS (for pediatric practice) within 180 days of start date.
