Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• Completion of ASHP-accredited PGY1 pharmacy residency or equivalent is desirable.
• Current Connecticut Pharmacist License or eligibility to licensure in Connecticut.
• Basic knowledge of pharmacology and therapeutics; strong verbal and written communication; ability to work with others.

Responsibilities

• Promote CHAMP behaviors and patient safety as a core value.
• Conduct timely and accurate medication order reviews considering patient characteristics including labs and vital signs.
• Demonstrate competency in pharmacy automation and technology as applicable to the work area.
• Adhere to departmental policies, procedures, protocols, and guidelines to ensure quality and safety.
• Provide education and training to pharmacy staff, students, residents, and healthcare team members.
• Contribute to departmental cost-reduction and revenue-generation efforts.
