Job summary

Eskenazi Health seeks a Clinical Pharmacist to join the hospital-based pharmacy team. You will participate in interdisciplinary rounds, perform medication reconciliation, and monitor therapy to optimize patient outcomes.

Your role requires ensuring regulatory compliance, training learners, and advancing safe, effective medication use. This position offers full-time hours with a varied day/evening schedule at the Indianapolis campus.

Qualifications

• PharmD degree or BS in Pharmacy with equivalent experience
• Indiana licensure or eligibility (licensure within 90 days of hire)
• Completion of ASHP-accredited PGY1 Pharmacy Residency required
• PGY2 Residency preferred
• Board certifications (BCPS, BCCCP, BCSCP) preferred
• BLS/ACLS certification as required by assignment
• Participation in continuing education for licensure/certification

Responsibilities

• Optimizes medication regimens for assigned patients with pharmacotherapy plans safely and effectively
• Performs therapeutic drug monitoring and documents care per standards
• Conducts medication history, reconciliation, education, and follow-up care coordination
• Assists with formulary management and drug-use policy
• Ensures accurate order entry, labeling, and preparation of medications
• Promotes safety culture and reports adverse drug events
• Teaches learners and supports onboarding and ongoing education
• Maintains regulatory compliance with Joint Commission, CMS, and ASHP standards
