Job summary

Option Care Health, a leading home infusion provider, is hiring a Clinical Pharmacist in Florida. This entry-level role supports safe and appropriate pharmacy services in the home care setting, coordinating with nursing and other care teams.

You will monitor patient progress, educate patients and caregivers, and ensure compliance with policy, standards, and regulatory entities while developing pharmacotherapy expertise for infusion patients.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Experience or training in sterile IV admixture or MTM is a plus.

Responsibilities

• Determines suitability of patients for home care and collects required information.
• Designs and monitors patient-specific pharmacy care plans for home care.
• Maintains medication safety, dosing, and education for patients and caregivers.
• Ensures continuity of pharmaceutical care across home and other settings.
• Complies with state/federal regulations and DEA guidelines.
