Job summary

AdventHealth in Littleton, CO, is seeking a Pharmacist for full-time day-weekend shifts. You will educate patients and families, oversee medication therapy and safety, and participate in interdisciplinary rounds to ensure high-quality transitions of care.

Responsibilities include supervising pharmacy staff, optimizing documentation, and supporting ongoing professional development. A PharmD, active pharmacist license, and hospital experience are preferred.

Qualifications

  • Pharmacist license (RPH) required.
  • Bachelor's degree required; Doctorate preferred.
  • 1+ hospital experience preferred.
  • PharmD as field of study.

Responsibilities

  • Educate and counsel patients, families, and healthcare team on safe medication use.
  • Provide safe, timely, cost-effective medication therapy and participate in stewardship, surveillance, and restricted-use programs.
  • Engage in direct patient care and interdisciplinary rounds; ensure smooth transitions of care from admission through discharge and follow-up.
  • Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development.
  • Identify, report, and help resolve medication safety events, adverse drug reactions, and errors.
  • Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.
  • Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
  • Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.

Job Description:

Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas. Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments. Other duties as assigned.Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
