Job summary

AdventHealth is hiring a licensed Pharmacist to join a full-spectrum clinical team in Denver, Colorado. You will educate patients and care teams on safe medication use and participate in stewardship and outcomes improvement.

The role requires a PharmD, a state Pharmacist license, and hospital experience. Expect direct patient care, rounds, and leadership of pharmacy staff across central, satellite, and IV operations.

Responsibilities

• Educate and counsel patients, families, and healthcare team members on safe and effective medication use.
• Provide safe, timely, and cost-effective medication therapy with stewardship and surveillance.
• Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission to discharge and follow-up.
• Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development.
• Identify, report, and help resolve medication safety events, adverse drug reactions, and errors.
• Demonstrate proficiency with EHRs, pharmacy informatics systems, and medication automation to optimize clinical decision support and documentation.
• Contribute to professional advancement through practice innovation, research, specialty alignment, committee participation, and scholarly activity.
• Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.

Job Description:

• Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up.
• Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development.
• Identify, report, and help resolve medication safety events, adverse drug reactions, and errors.
• Demonstrate proficiency with EHRs, pharmacy informatics systems, and medication automation to optimize clinical decision support and documentation.
• Contribute to professional advancement through practice innovation, research, specialty alignment (e.g., critical care, oncology, infectious disease), committee participation, and scholarly activity.
• Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.
• Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.
• Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
• Performs other duties as assigned.
