Responsibilities

• Provide patient-specific therapeutic drug monitoring and communicate findings to care team.
• Perform medication reconciliation at transitions of care and maintain accurate medication profiles.
• Serve as a drug information resource and review orders for appropriateness, dosage and interactions.
• Assist with recalls, formulary decisions, and emergency preparedness as needed.
