Job summary

Axil Health is seeking a Clinical Pharmacist to lead CCM, TCM, and RPM services across independent clinics in North Carolina under a Collaborative Practice Agreement. You will build patient panels, enroll patients, interpret RPM data, and collaborate on medication management with supervising providers.

This role involves precepting PGY-1 residents, developing individualized care plans, and implementing therapy changes as needed, with travel to multiple clinics within commutable distance.

Qualifications

• Must hold a current, valid pharmacist license to practice in North Carolina.
• Current CPR certification.
• Certification for Immunization.
• PharmD with three years of experience or Board Certification/Residency equivalences is preferred.

Responsibilities

• Lead CCM, TCM, and RPM services under a Collaborative Practice Agreement.
• Build and manage patient panels; enroll patients and interpret RPM data.
• Collaborate on medication management with supervising providers.
• Precept PGY-1 students and participate in our Fellowship Program.
• Develop individualized care plans and implement therapy changes when needed.
