Description

The Clinical Pharmacist is responsible for identifying, preventing, and resolving medication-related problems; providing authoritative information to healthcare professionals, patients, and the public; and ensuring that pharmacy services function as an integrated system to deliver patient-centered care.

The mission of the Department of Pharmacy is to deliver safe, high-quality, effective, and efficient patient care, develop personnel, advance research, and coordinate pharmacy services across Northwestern Medicine.

Responsibilities
  • Interpret medication orders to ensure safety and proper use, coordinate problem-solving among pharmacy, nursing, and prescribers, demonstrating initiative and sound judgment.
  • Monitor medication therapy for interactions and intervene when necessary.
  • Evaluate and document medication appropriateness, communicate potential issues and solutions to prescribers.
  • Consult on pharmacokinetics and adverse reactions, participate in therapeutic drug monitoring, and adjust therapy accordingly.
  • Process medication orders into the Electronic Health Record accurately.
  • Maintain comprehensive patient pharmacy profiles.
  • Oversee medication packaging, labeling, and distribution, monitor controlled substances, and maintain inventories.
  • Compound and dispense pharmaceuticals, including sterile and chemotherapy products.
  • Verify medication orders, ensure accuracy, and resolve discrepancies.
  • Review laboratory data for medication appropriateness and recommend changes.
  • Develop and deliver training for staff and students.
  • Educate patients and families about medications.
  • Respond to emergency medication management situations.
  • Provide medication information and education to healthcare professionals and patients.
  • Maintain the hospital formulary and ensure medication use complies with it.
  • Document clinical activities and interventions, collect data, and maintain records.
  • Communicate effectively with patients and healthcare professionals.
  • Participate in quality improvement activities.
  • Prioritize and organize workload, oversee pharmacy staff activities, and ensure departmental objectives are met.
  • Review and resolve issues with automated dispensing cabinets.
  • Ensure compliance with hazardous waste procedures.
  • Maintain professional competency through ongoing development.
  • Perform other duties as assigned.

Qualifications

Required:

  • Current Illinois pharmacy license
  • Bachelor’s or Doctor of Pharmacy degree

Preferred:

  • Specialty training (neonatology, critical care, oncology, etc.)
  • Completion of a pharmacy residency or equivalent hospital experience
