The Clinical Pharmacist - Anticoagulation Stewardship supports inpatient pharmacy services across multiple hospitals in the Florida market, serving as a clinical expert and liaison to promote safe, standardized anticoagulation practices.

Qualifications

• PharmD or BS in Pharmacy from an accredited college of pharmacy.
• Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation within 90 days of hire.
• BLS certification through AHA or Red Cross and completion of a Board of Pharmacy approved immunizations course, or certification obtained within 90 days of hire.
• ACLS or PALS certification through AHA or Red Cross (or within one year of hire).

Responsibilities

• Provide safe, efficient, patient-centered and evidence-based pharmacy and medication services.
• Ensure appropriate selection, dosing, administration and dispensing of medication orders.
• Verify medication orders and/or dispense medications accurately according to state and federal regulations.
• Document services in a complete, accurate and timely manner.
• Actively contribute to medication safety initiatives.
• Ensure all pharmacy operations are in compliance with national accreditation standards.
• Educate and train students, residents, other caregivers, and/or patients.
• Provide cost-effective care.
• Be accountable for drug use policy adherence.

Education

PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy

Basic Life Support (BLS) certification through AHA or Red Cross

Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification
