Job summary

Virginia Mason Franciscan Health seeks a Pharmacist to support doctors and nurses in safely prescribing and dispensing medications. You will review prescriptions for safety, assess for drug interactions, dosing accuracy, allergies, and finalize before dispensing to the nursing station.

You will collaborate with healthcare providers, support care transitions, and strive to deliver exceptional patient experiences with high-quality care.

Qualifications

  • Graduation from an accredited school of pharmacy (upon hire).
  • WA Pharmacist license required (upon hire).
  • Doctor of Pharmacy (PharmD) preferred.
  • Three years of ambulatory care experience or PGY2 ambulatory care residency preferred.
  • PGY1 with one year ambulatory care experience preferred.

Responsibilities

  • Participates in evaluation and management of diseases and health conditions in collaboration with other health care providers.
  • Prescribes pharmaceutical agents as outlined in the approved CDTM agreement, including initiating, adjusting, and discontinuing drug therapy.
  • Orders and interprets drug-therapy related tests.
  • Ensures patients have access to safe, efficacious, and cost-effective medication therapy.
  • Works collaboratively to resolve medication access and adherence issues.
  • Addresses medication-related issues to support care coordination and transitions of care.

Education

Doctor of Pharmacy / PharmD
Pharmacist license (WA)
Ambulatory care residency or experience (preferred)
PGY1/PGY2 residency (preferred)
Collaborative drug therapy management (CDTM) experience (preferred)

Job description

Job Summary and Responsibilities

As our Pharmacist, you’ll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury.

Every day you will meticulously review every prescription in your queue for safety, critically assessing for potential drug interactions, correct dosing, patient allergies, or any irregularities. You'll then authorize prescriptions for the pharmacy technician, performing a final check before dispensing medication to the nursing station, ensuring expert oversight in every process.

To excel in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for real people. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.

Job Requirements
Required
  • Graduation from an accredited school of pharmacy, upon hire and
  • Pharmacist: WA, upon hire
Preferred
  • Doctor Of Pharmacy, upon hire and
  • Completion of three (3) years of practice experience with at least 50% of time spent in ambulatory care activities or
  • Completion of a PGY2 Ambulatory Care Pharmacy residency or
  • Completion of a PGY1 residency plus one (1) year of practice experience with at least 50% of time spent in ambulatory care activities
