Job summary

Trinity Health is seeking a part-time Clinical Pharmacist, Ambulatory Care in Grand Rapids, MI. The role focuses on drug therapy management for outpatient patients, participating in CPAs, and improving pharmacotherapy effectiveness with physicians and the care team.

The position includes time in the Anticoagulation Management Service and training of pharmacy residents and students. Flexible days; Monday–Friday schedule.

Qualifications

• PharmD from ACPE-accredited school/college required.
• PGY-1 required; PGY-2 Ambulatory Care preferred or commensurate experience.
• BPS Pharmacy Specialty Certification preferred or eligible within 1 year.

Responsibilities

• Provide clinical pharmacy services for ambulatory patients.
• Conduct drug therapy management and patient assessments.
• Participate in established programs (formulary, education, quality improvement).
• Assist in anticoagulation management service and other initiatives.
• Precept pharmacy residents and students as assigned.
• Educate patients and staff on drug therapy and safety.

Position Summary: Clinical Pharmacist, Ambulatory Care

Responsible for providing clinical pharmacy services for outpatient patients by providing drug therapy management and assessment for patients in the practice area. Provides support for established programs including, but not limited to, drug formulary, pharmacy education programs, and quality improvement and research relating to drug therapy.

What you will need:

• Education/Training: Doctor of Pharmacy degree (PharmD) from an ACPE-accredited school/college of Pharmacy (required), PGY-1 Residency (required), PGY-2 Ambulatory Care Residency (strongly preferred) or commensurate practice experience in ambulatory care with direct pharmacotherapy management responsibilities; BPS Specialty Certification (Pharmacotherapy or Ambulatory Care, preferred; or eligible with certification within 1 year of hire)
• Licensure: Licensed (pharmacist and controlled substance) in the State of Michigan, or eligible
• Description: This is a part-time (40 hours per pay-period), salaried, Monday thru Friday (days can be flexible) position for a Clinical Pharmacist, Ambulatory Care within the Trinity Health Medical Group primary care, PCMH offices (part of the Trinity Health Saint Mary’s health-system in Grand Rapids, Michigan). Responsibilities include disease management within institution approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation, others), referral-based Comprehensive Medication Review with recommendations to physicians, daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost as well as participating in team-based care within a multidisciplinary model. Additional activities include time in our Anticoagulation Management Service (approximately 1 day/week), and other multidisciplinary initiatives focused on particular high-risk patients or populations (e.g., depression, frail elderly) (approximately ½ day/week). Participation in the training of Trinity Health Grand Rapids PGY-1 & PGY-2 Pharmacy residents and PharmD students from the College of Pharmacy at Ferris State University is also expected as is participation in both medical group and/or health system committees.

• Provides drug therapy management and assessment.
• Identifies and resolves potential and actual drug related problems.
• Assists physicians in designing dosing regimens based on patient-specific dosing parameters.
• Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines.
• Provides support to the Pharmacy & Therapeutics Committee, which may include serving as a member and/or activities such as preparing formulary drug reviews, medication usage evaluations, policies and guidelines for drug therapy, etc.
• Participates in performance improvement initiatives and research as appropriate.
• Participates in departmental, multidisciplinary, hospital and health-system committees as leader, member or consultant.
• Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.
• Provides education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students and other clinicians.
• Performs other related duties as assigned such as quality assurance reports, preparation for regulatory inspections, etc.
