Qualifications

  • Bachelor of Science in Pharmacy required.
  • PharmD or advanced degree preferred.
  • Licensed or eligible to be licensed in Michigan.
  • ASHP-accredited residency (PGY-1/PGY-2) preferred.

Responsibilities

  • Provide direct patient care: develop, monitor, and adjust pharmacotherapeutic regimens; participate in rounds with physicians and nurses to optimize outcomes.
  • Deliver clinical pharmacy services: provide drug information, therapeutic recommendations, and support antimicrobial stewardship and other initiatives as assigned.
  • Staffing: rotate between clinical and centralized operations; day and evening shifts 6:00 a.m.–11:00 p.m.; occasional overnight coverage; perform order verification and dispensing.
  • Medication preparation & dispensing: accurately compound, package, label, and dispense medications and IV solutions per USP <797>/<800>.
  • Supervision & delegation: oversee technicians and interns and ensure proper task delegation.
  • Education & mentorship: act as a resource and preceptor for students and residents; contribute to training programs.
  • Quality improvement & professional development: participate in department projects and pursue ongoing growth.

Description:
POSITION SUMMARY:

The Inpatient Clinical Pharmacist is an integral member of the interdisciplinary healthcare team, responsible for optimizing medication therapy and ensuring safe, effective, and patient-centered pharmaceutical care. This role combines direct clinical service, operational pharmacy support, education, and continuous quality improvement.

KEY RESPONSIBILITIES
  • Provide Direct Patient Care:
    Develop, recommend, and monitor individualized pharmacotherapeutic regimens using expertise in pharmacology, pathophysiology, and pharmacokinetics. Participate in patient care rounds and collaborate closely with physicians, nurses, and other healthcare professionals to ensure optimal medication outcomes.
  • Deliver Clinical Pharmacy Services:
    Offer timely drug information, therapeutic recommendations, and evidence-based guidance to the medical staff. Support antimicrobial stewardship, anticoagulation management, and other clinical initiatives as assigned.
  • Staffing Responsibilities:
    Rotate through decentralized clinical pharmacy services and centralized pharmacy operations. This position primarily rotates between department day and evening shifts hours of 6:00 a.m. and 11:00 p.m. Occasional overnight coverage may be requested; such instances are infrequent. Perform order verification, medication distribution, and sterile and non-sterile compounding in accordance with hospital policies and regulatory standards.
  • Medication Preparation & Dispensing:
    Accurately compound, package, label, and dispense medications, IV solutions, and specialized preparations. Ensure compliance with USP <797>/<800> and maintain a high standard of safety and quality.
  • Supervision & Delegation:
    Provide oversight, guidance, and support to pharmacy technicians and pharmacy interns in daily operations. Ensure appropriate task delegation and adherence to workflow standards.
  • Education & Mentorship:
    Serve as a resource and preceptor for pharmacy students, residents, and learners from other health professions. Contribute to the development of educational programs and training activities.
  • Quality Improvement & Professional Development:
    Participate in departmental performance improvement projects and hospital quality initiatives. Pursue ongoing professional growth to meet organizational, departmental, and personal advancement goals.
POSITION QUALIFICATIONS
  • Education & Licensure
    • Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) or other advanced degree preferred.
    • Must be licensed, or eligible for licensure, as a pharmacist in the State of Michigan.
    • Strongly preferred: completion of an ASHP-accredited residency (PGY-1 and/or PGY-2)

    • Strong verbal and written communication skills with the ability to convey clinical information clearly and compassionately.

    • Ability to identify medication-related issues and patient care concerns, recognize situations outside the pharmacist's scope of responsibility, and initiate appropriate follow-up or escalation.

    • Demonstrated flexibility in adjusting work priorities in collaboration with supervisors based on patient care needs, staffing demands, or departmental changes.
