Job summary

Qualifications

• Doctor of Pharmacy (PharmD) degree required and licensed in state; graduate of accredited school.
• BLS certification required; ACLS certification within 12 months of hire.
• Bilingual English and Navajo preferred.

Responsibilities

• Maintain regular attendance and punctuality.
• Provide pharmaceutical services including medication review and therapeutic drug monitoring.
• Counsel patients on medication use, storage, risks and side effects; document activities in medical records.
• Collaborate with physicians and other clinicians to develop treatment protocols and perform clinical reviews of medications.
