Job summary

HCA Florida Trinity Hospital is seeking a Clinical Pharmacist PRN to deliver clinical pharmacy services and ensure safe medication management across patient areas. You will support program development and collaborate with the healthcare team to optimize patient outcomes.

Key duties include reviewing orders, ensuring proper dosing, monitoring controlled substances, and providing drug information to physicians.

Qualifications

Bachelor of Science or Pharm D degree in Pharmacy from College of Pharmacy.
Experience as a pharmacist in an acute-care hospital setting or general pharmacy practice residency preferred.
Current FL Pharmacist license or license eligible.
Eligible to sit for North American Pharmacist Licensure Exam (NAPLEX).
Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills.

Responsibilities

Coordinate and assist with development, implementation, and maintenance of clinical pharmacy programs.
Provide clinical pharmacy support and execute programs for all patient areas.
Review medication orders and ensure appropriateness of drugs, dosage, route and interactions; adjust as needed.
Ensure medications are safely dispensed to patients in accordance with age.
Close monitoring of narcotic transactions and reconciliation of dispensed substances.
Assist department with physician consults and provide reliable drug information.
Communicate effectively with physicians and the interdisciplinary team to meet patient needs.
