Position Summary

Responsible for attending to the medication needs of the patients and provide drug information to patients, pharmacists, physicians, nurses and other hospital staff.

Education

• Doctorate of Pharmacy (Clinical)

Licenses and Certification

• Illinois license of Pharmacy
• (225 ILCS 85/6 - 225 ILCS 85/8; 68 IL ADC 1330.300 - 68 IL ADC 1330.350) States: "An applicant may work as a registered pharmacist for up to 60 days prior to the issuance of a certificate of registration upon receipt of a notice from the Division that the examination was successfully completed."

Role Specific Responsibilities

• Maintains records, processes and monitors narcotic usage.

• Monitors drug therapy, takes an active role in drug utilization , intervenes on the patients behalf as necessary, including conferring with providers and other caregivers about therapy, and documents those interventions.

• Obtains and maintains competence in pharmacy, including assigned areas and required continuing education
• Participates in work assignments at other SIH facilities.
