Job summary

VIVA USA Inc. seeks a remote Pharmacist to perform medication therapy management (MTM) and comprehensive medication reviews (CMR) for patients using a third‑party web application. You will communicate with patients via outbound calls and document MTM findings per guidelines.

Requirements include a valid FL Pharmacist License and at least 4 years of MTM experience. Strong communication skills and tech proficiency, including Salesforce familiarity, are essential.

Qualifications

• Education/Licensing: Valid Pharmacist License in FL.
• Clinical MTM experience: at least 4 years as MTM pharmacist (management preferred).
• Familiar with Part D Star measures: SUPD, SPC, CMR.
• Pharmaceutical/medical vocabulary proficiency.
• Experience in medication review.

Responsibilities

• Outbound calls for MTM and CMR (approx. 100 calls daily).
• Conduct comprehensive medication reviews (CMR) and MTM services.
• Transcribe prescription/diagnosis information into a third‑party web app.
• Enter data accurately into the system to generate standardized patient documents.

Position Overview:
The pharmacist will be responsible for medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines. The pharmacist will transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application. The data to be transcribed will be either hard-copy or available from client application. The data entered by the pharmacist will be used to create a standardized document that will be mailed to individual patients. The pharmacist is a member of the MTM product team.

Qualifications

• Education/Licensing Requirements: Valid Pharmacist License in FL
• At least 4 years of experience as clinical MTM pharmacist. (Management experience preferred)
• Familiar with driving Part D Star measures. Specifically, SUPD, SPC, and CMR.
• Understanding of pharmaceutical and medical vocabulary
• Clinical experience in medication review.
