Job summary

Portneuf Medical Center in Pocatello, ID is seeking a night shift, full-time Pharmacist to join our hospital team. You will provide comprehensive pharmaceutical care, oversee technicians, and support patient safety initiatives in a high-volume setting.

The role requires a PharmD or BS with ACPE approval, hospital experience, and active licensure. You will participate in emergencies and mentor students, contributing to excellent patient outcomes and interprofessional collaboration.

Qualifications

  • B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
  • Clinical pharmacy experience in a hospital setting.
  • Completion of an accredited PGY-1 Pharmacy residency or fellowship - OR - 2+ years of clinical pharmacy experience with no residency.
  • Active Pharmacist license in good standing with the State Board of Pharmacy.
  • BLS certification obtained within 14 days of hire.
  • ALS/ACLS certification within 90 days of hire.

Responsibilities

  • Optimize patient care with safe, cost-effective pharmaceutical services including drug dispensing and IV admixture.
  • Oversee pharmacy technicians, students and support staff under supervision.
  • Interpret orders and verify them in the pharmacy system.
  • Accurately compound and dispense pharmaceuticals.
  • Monitor drug therapy for interactions, allergies and appropriateness.
  • Provide timely medication information to staff and patients.
  • Interact with medical staff to resolve drug therapy issues.
  • Document clinical activities and interventions clearly.
  • Maintain formulary and minimize non-formulary procurements.
  • Lead and supervise staff to meet medication turnaround time policies.
  • Serve as a preceptor for pharmacy students or residents.
  • Support quality improvement, risk management and patient safety programs.

  • Portneuf Medical Center (PMC) is a nationally recognized healthcare leader with 205 beds, serving as the region's Level II Trauma Center and tertiary referral hospital, and is home to the Portneuf Heart and Vascular Institute, Portneuf Cancer Center, Portneuf Medical Group, and Portneuf Air Rescue. Responsibilities
  • Optimizes patient care by providing appropriate, safe, and cost effective pharmaceutical care including drug dispensing, IV admixture program, and providing drug information.
  • Works under the direction of the Director of Pharmacy and/or the Pharmacy Operations Manager and will oversee the work of pharmacy technicians, students, and support staff.
  • Interpret orders and transcribe or verify them accurately and efficiently in the pharmacy computer system.
  • Accurately compound and dispense pharmaceuticals.
  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
  • Provide accurate and timely medication information to healthcare staff and patients.
  • Effectively and proactively interact with medical staff regarding drug therapy issue resolutions.
  • Document all clinical activities and interventions accurately and completely.
  • Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols and/or guardrails.
  • Supervise and assist pharmacy technicians and other support staff effectively.
  • Prioritize work and lead others to comply with facility medication turn-around time policy and procedure and identify areas for improved efficiency.
  • Must serve as preceptor for pharmacy students and/or residents.
  • Support and participate in the goals of the organization’s quality improvement, risk management, and patient safety program to include but not limited to National Patient Safety Goals, Core Measures, and HCAHPS medication related initiatives.
  • Maintain appropriate controlled substance records.
  • Make appropriate evidence-based, patient-centered medication recommendations.
  • Participate in the management of medical emergencies as appropriate, including, but not limited to Code Blue.
  • Provide clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
Qualifications

Job Requirements:

  • B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
  • Clinical pharmacy experience in a hospital setting.
  • Completion of an accredited PGY-1 Pharmacy residency or fellowship - OR- 2+ years of clinical pharmacy experience with no history of residency or fellowship.
  • Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
  • Other applicable state requirements – i.e., sterile compounding, etc.
  • BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.
  • ALS/ACLS certification (within 90 days of hire; ALS/ACLS required for attending Code Blue)

Preferred Job Requirements:

  • Doctor of Pharmacy from an ACPE-accredited college of pharmacy
