Responsibilities
  • Selects, compounds, dispenses and preserves drug therapy and other therapeutic agents as prescribed by an authorized medical practitioner in compliance with established standards and appropriate age.
  • Reviews patient chart for drug sensitivities, drug interactions, incompatibilities, dosage, and therapy treatment in accordance with patient diagnosis and conditions.
  • Meets with patient, patient family, and/or responsible party to verify understanding of medication, potential drug interactions, possible side effects, storage, and instructions regarding usage; documents in patient chart.
  • Organizes and maintains drug information sources; responds to questions/requests from medical, nursing, and other hospital staff.
  • Prepares sterile products for patient administration; determines proper preparation, packaging, sterilization, storage, and stability requirements of the product.
  • Researches pharmacological information as needed to ensure appropriateness of treatment.
  • Performs unit dose distribution, intravenous preparation, additive, and clinical rounds as a member of the healthcare team; responds to emergency situations by assisting with drug therapy.
  • Serves as therapeutic consultant to medical staff to ensure medication therapy is rational.
  • Actively participates in infection control, patient care policy formulation, and drug selection committees.
  • Prepares lectures and demonstrations for community members and staff; assists in the training program of pharmacy students.
  • Periodically inspects drugs in clinics and storerooms for adequacy, overstocking, arrangement, storage, proper rotation, and evidence of deterioration.
  • Performs primary care of patients with stable chronic conditions requesting medication refills and patients with acute self‑limiting diseases in accordance with established standards of care.
  • Maintains stock control records, narcotic records, and inventories to ensure compliance with legal and administrative requirements.
  • Participates in development of department policies, program planning, and coordination with other agencies in carrying out pharmacy functions.
  • Participates in departmental orientation, on‑the‑job training, and quality assurance programs/initiatives.
  • Participates in a variety of departmental and hospital educational programs to maintain current skill and competency levels; identifies and discusses performance or training needs with supervisor.
  • Performs other duties as requested.
Qualifications
