About The Job

Provides a variety of pharmacy services (inpatient and outpatient infusion), including therapeutic drug monitoring, drug regimen review, patient and staff education, medical staff interaction, resident teaching, and distribution pharmacist duties under the general direction of the Pharmacy Director or other Pharmacy Leadership.

Requirements

• Education: PharmD degree preferred, or B.S. in pharmacy, or successful completion of the foreign pharmacy equivalency exam. Residency is preferred but not required.
• Licensure: Valid Indiana License or graduate of an accredited pharmacy school with Indiana intern license.
• Experience: Proficiency in pharmacy practice, especially pharmacokinetic monitoring, nutritional consults, and drug information services, usually obtained from graduation or a pharmacy residency, with at least two years of hospital/institutional pharmacy experience or completion of a pharmacy practice residency.
