Clinical Pharmacist

The Clinical Pharmacist works directly with patients to ensure their care plan, including medications, contributes to the best outcomes. They operate in a clinical environment, collaborating with physicians, advanced practice providers, and other healthcare professionals from both AndHealth and our partners, facilitating improved care coordination through a Collaborative Practice Agreement.

As an active participant in each patient’s health journey, they apply their specialized knowledge within their scope of practice to assess healthcare needs, optimize prescriptions, advise on medication use, and support care team efforts to educate on other health drivers like exercise, nutrition, and supplements.

Responsibilities

1. Provide a “Wow” patient experience during every interaction.
2. Deliver clinical pharmacy and patient care services, including medication management, adverse effects, and drug interactions via a collaborative practice agreement (CPA).
3. Administer medications following company protocols.
4. Assess patient health needs, collaborate with providers to adjust treatment plans for optimal outcomes.
5. Stay updated with clinical guidelines and monitor patient progress.
6. Identify untreated health issues and develop appropriate treatment plans.
7. Educate patients on medication use and reinforce lifestyle modifications.
8. Participate in patient panel reviews and escalations as per CPA.
9. Ensure pharmacy compliance with policies, quality standards, and regulatory requirements; assist with QA tasks for federal programs and accreditations.
10. Travel locally/regionally as needed to meet partner and company needs.
11. Perform other duties as required to meet business needs.

Qualifications

• Education: PharmD or equivalent; FPGEC (NABP) required.
• Licensure: Unrestricted Indiana pharmacist license; willingness to obtain multi-state licenses.
• Certifications: BLS/CPR endorsed by AHA; NASP CSP preferred; Immunization Certificate preferred.
• Experience: 3+ years outpatient care preferred; 2+ years specialty pharmacy strongly preferred; PGY1 or equivalent experience preferred.
• Knowledge of pharmacy laws, standards, and quality principles.
• Excellent communication skills, ability to simplify complex concepts.
• Proficiency with EMR/EHR systems (Epic, Cerner, Meditech, etc.).
• Patient-focused, collaborative, and technologically adept.
• Trustworthy with confidential information.
