Job summary

HCA Houston Healthcare Medical Center in Houston, TX seeks a Clinical Pharmacist to support patient care in a busy acute care setting. You will provide clinical pharmacy services, collaborate with physicians and staff, and contribute to medication safety and outcomes.

PharmD preferred with Texas license; 2 years hospital experience ideal. Strong communication, leadership and teaching skills, plus proficiency in Word, Excel and Access, are essential.

Qualifications

  • PharmD preferred.
  • Current Texas Pharmacist license.
  • 2 yrs clinical experience in an acute care hospital setting preferred.

Responsibilities

  • Provide clinical pharmacy/pharmaceutical care for hospital patients.
  • Maintain current with clinical literature related to hospital patient populations.
  • Maintain competency to provide pharmaceutical care to all ages and types of patients.
  • Serve as pharmacy representative for weekly team conference meetings.
  • Process automatic stop order forms.
  • Monitor patients on anticoagulant and antibiotic therapy and make physician recommendations.
  • Perform DUE as needed.
  • Serve as member of P&T committee and present reports as needed.
  • Oversee ADR program.
  • Provide drug information to hospital staff, administration and physicians.

Job Summary and Qualifications
Clinical responsibilities:
  • Provide clinical pharmacy/pharmaceutical care for hospital patients;
  • Maintain current with clinical literature especially related to the hospital-specific patient populations;
  • Maintain competency to provide pharmaceutical care to all ages and types of patients that may be maintained within this hospital;
  • Serve as pharmacy representative for weekly team conference meetings;
  • Process automatic stop order forms;
  • Monitor patients on anticoagulant therapy, antibiotic therapy and make recommendations to physicians as needed;
  • Perform DUE as needed;
  • Serve as member of P&T committee and present reports as needed;
  • .oversee ADR program;

Leadership and Supervisory responsibilities
  • 1. provide pharmacy students and residents guidance and education;
  • 2. help provide annual goals for clinical program;
  • 3. provide supervision of pharmacy technicians as needed
What qualifications you will need:

Skills and Knowledge

Effective skills in regards to communication, presentation, leadership and teaching; proficient computer skills including Microsoft Word, Excel and Access.

Physical Demands

Frequently(at least 50%) - Standing/Walking. Usually(25-49%) - Sitting, Bending/Stooping, Using leg muscles frequently or for extended periods, Using back muscles frequently or for extended periods. Weight carried or lifted - 2-10 pounds and 11-20 pounds. Infrequently(less than 25%) - Turning, Lifting/Carrying, Pushing/Pulling, Climbing/ascending/descending ladders/stairs, Using arm muscles frequently or for extended periods, Carried or Lifted - 41-50 pounds and 51 pounds or more requires help. Working with or near chemicals, Working near radiation sources, Working with hazardous waste materials, Operating vehicles or machinery and Wearing protective clothing and/or equipment.

Working Conditions

Challenging work environment requiring good multi-tasking skills.

Close interaction with customers: patients, physicians, administration and nursing & other departmental staff members

May need to work more than 40 hrs a week and alternate shifts, when necessary
