Job summary

Qualifications

Role Description

The Clinical Pharmacist is a full-time, on-site role based in Houston, TX. This position is responsible for overseeing clinical pharmacy services related to infusion and specialty medications, ensuring safe, effective, and evidence-based pharmaceutical care for patients. Day-to-day responsibilities include reviewing and verifying medication orders, performing clinical assessments, monitoring therapy outcomes, and collaborating with physicians, nurses, and other healthcare professionals to optimize treatment plans. The Clinical Pharmacist will provide patient and caregiver education on medication use, side effects, and adherence, and support home infusion regimens by coordinating dosing and monitoring requirements. Additional duties include maintaining regulatory and accreditation standards, participating in quality improvement initiatives, and assisting with formulation, compounding, and proper storage of medications used in infusion therapy.

Qualifications
  • Candidates should possess strong skills in Pharmacy Practice and Pharmacy, with the ability to manage infusion and specialty medication therapies safely and effectively.
  • Candidates should possess skills in Pharmaceutical Care and Medicine, including clinical assessment, therapy monitoring, and interprofessional collaboration for complex treatment plans.
  • Candidates should possess skills in Compounding, including preparation of sterile and non-sterile products in accordance with USP standards and organizational policies.
  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy; completion of a PGY1 residency in clinical or hospital pharmacy is preferred.
  • Current, unrestricted pharmacist license in the state of Texas; board certification in a relevant specialty (e.g., BCPS, BCOP) is an advantage.
  • Experience in infusion pharmacy, hospital pharmacy, or specialty pharmacy, with familiarity in home infusion protocols and sterile compounding.
  • Strong communication, documentation, and patient education skills, with the ability to work collaboratively in a multidisciplinary team.
  • Commitment to patient-centered care, adherence to regulatory and safety standards, and ongoing professional development.
