The Clinical Pharmacist is a full-time, on-site role based in Houston, TX. This position is responsible for overseeing clinical pharmacy services related to infusion and specialty medications, ensuring safe, effective, and evidence-based pharmaceutical care for patients. Day-to-day responsibilities include reviewing and verifying medication orders, performing clinical assessments, monitoring therapy outcomes, and collaborating with physicians, nurses, and other healthcare professionals to optimize treatment plans. The Clinical Pharmacist will provide patient and caregiver education on medication use, side effects, and adherence, and support home infusion regimens by coordinating dosing and monitoring requirements. Additional duties include maintaining regulatory and accreditation standards, participating in quality improvement initiatives, and assisting with formulation, compounding, and proper storage of medications used in infusion therapy.
