Job description

Role: Clinical Pharmacist I

Responsibilities: Verify and dispense medications, perform clinical medication reviews, collaborate with healthcare professionals to optimize pharmacotherapy, conduct medication reconciliation, monitor therapeutic drugs, support antimicrobial stewardship, and provide patient counseling on medication use and adherence.

Key Duties: Document clinical interventions in electronic health records, contribute to quality and safety initiatives, and participate in educational activities for patients, staff, and learners.
