Qualifications

Responsibilities

• Maintain an assigned caseload of participants with attention to acuity and coverage.
• Conduct nursing assessments as required by regulations and update the plan of care.
• Participate in IDT and coordinate nursing care and discharge planning.
• Provide nursing services within RN scope and ensure documentation in EHR.
• Support quality and compliance activities and regulatory monitoring.
