Job summary

Qualifications

• Currently enrolled in final year of Dietetics/Nutrition program.
• Must be able to work the hours specified and communicate effectively.

Responsibilities

• Performs nutrition screening in a timely manner and refers high risk patients to dietitians.
• Performs nutrition evaluation for low to moderate risk patients including gathering nutritional needs.
• Develops and implements individualized nutrition care plans with measurable goals.
• Evaluates responses to care and revises plans; refers to dietitians as needed.
• Provides nutrition education to patients and families based on evaluation of needs.
• Documents nutrition care in the EMR accurately and timely.
• Assists patients with menu choices as needed.
