Job summary

Manchester Center for Rehabilitation and Healing seeks a Clinical Nutrition Manager / Registered Dietitian to lead the clinical nutrition program within a skilled nursing and rehabilitation setting in Manchester, TN. You will oversee dietetic staff, perform comprehensive nutrition assessments, and develop individualized care plans to support residents' health and recovery.

Ideal candidates hold RD/RDN credentials and a Bachelor's in Dietetics or Nutrition, with experience in LTC or rehab.

Qualifications

RD/RDN required or preferred per state/facility requirements
Bachelor's degree in Dietetics, Nutrition, or related field
Experience in skilled nursing, rehab, hospital, or long-term care setting preferred
Strong knowledge of clinical nutrition and therapeutic diets
Experience with MDS and regulatory requirements in LTC
Strong clinical assessment, communication, and organizational skills
Ability to work with interdisciplinary team
Compassionate, resident-focused care
Strong leadership and problem-solving abilities

Responsibilities

Oversee the clinical nutrition program and deliver high-quality resident-centered nutrition care.
Complete and oversee comprehensive nutritional assessments for residents.
Develop individualized nutrition care plans based on medical conditions, dietary needs, and goals.
Monitor residents at nutritional risk and those with weight loss or wounds.
Collaborate with Nursing, Physicians, Speech Therapy, and Rehab teams.
Participate in care plan meetings and provide nutrition recommendations.
Monitor weights, intake, labs, and other clinical indicators.
Recommend therapeutic diets, supplements, and nutrition interventions.
Communicate nutrition concerns to residents, families, and staff.
Supervise and train dietary/nutrition team members.
Ensure compliance with federal, state, and facility regulations.
Assist with survey readiness and accurate clinical documentation.
Review nutritional outcomes and adjust interventions as needed.
Educate staff on nutrition, diets, and allergies.
Coordinate with Dietary Department to meet residents' needs.
Participate in QAPI initiatives related to nutrition and dining services.
