Job summary

Qualifications

• Graduation from an accredited Nursing program; BSN preferred.
• Current NYS RN license.
• BCLS and ACLS certifications as listed.
• RN may be matriculated in a BSN program with graduation date within the year.

Responsibilities

• Delivers patient care using the nursing process and Relationship Centered Care.
• Assesses and evaluates patient care needs using critical thinking.
• Collaborates with interdisciplinary team to plan and evaluate care.
• Prioritizes and delegates care within the Care Delivery Model.
• Educates patients and caregivers on plan of care, transition and prevention.
• Communicates effectively with patients, families and care team.
