Job summary

LiveWell Homecare Agency, LLC seeks an experienced Registered Nurse to provide direct patient care in a rehabilitation unit for adults and older adults requiring intensive therapies such as speech, occupational, and physical therapy.

You will develop, implement, and document individualized care plans, educate patients and families, and ensure safety and regulatory compliance while communicating across care transitions in a team-oriented environment.

Qualifications

• RN license required.
• Nursing diploma or associate degree; bachelor’s preferred.
