The role you’ll contribute:
The Nurse Navigator coordinates services required to provide multidisciplinary, quality-assured, appropriate case selection, and cost-effective care to all cardiovascular patients. This role serves as a resource for patients, physicians, and others, acting as an educator, quality improver, and consultant. Makes contact with the patient/family to establish a professional relationship and understanding of the diagnosis, treatment, and follow-up regimen. The nurse assesses the patient's status and works with physicians to determine the appropriate plan for the patient, ensuring compliance with regulatory and vendor guidelines. Develops, manages, and redesigns processes for scheduling procedures through discharge for patients in the CV department. Actively participates in outstanding customer service and maintains respectful relationships with all.

Develops, plans, and implements evidence-based measures to increase the quality of care across the cardiovascular spectrum and ensures compliance with all regulatory guidelines.
Supports data collection for process and quality improvement, understanding department performance trends and standards. Collaborates with the CV Quality department.
Maintains a database of patients meeting criteria for specific quality metrics and works with cardiologists to ensure criteria are met.
Reports findings to Cardiology Service Leaders and participates in committee meetings as required.
Keeps managers and directors informed, seeking assistance when unfamiliar procedures or techniques are encountered to ensure safe, effective patient care.
Coordinates patient education and ongoing care, serving as a resource during diagnosis and treatment. Communicates patient status and needs, collaborating with providers to meet needs and sequence care appropriately. Assesses patients’ learning styles and health literacy to tailor education.
Navigates patients from diagnosis through treatment, ensuring timely medical/supportive services.
Collaborates with multidisciplinary teams to ensure high-quality care and acts as a liaison with physicians’ offices regarding orders, records, consents, and documentation for procedures.
Facilitates discharge and follow-up processes, maintains professional relationships, and completes work timely and thoroughly.
