Overview

Job Title: Clinical Nurse - Navigator

Location: Special Procedures- Bone Marrow Transplant

Schedule: Days Monday–Friday,

Position Overview:

The Clinical Nurse - Navigator coordinates the care needs of assigned patients and develops comprehensive care plans to effectively manage care delivery across the patient care continuum. The Navigator partners with patients and their primary physicians to create customized care plans tailored to individual needs and preferences. They collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure timely delivery of tests and treatments. The Navigator advocates for the patient, balancing care needs with financial considerations to ensure efficient and effective treatment.

Key Responsibilities:

• Coordinate patient care and develop personalized, comprehensive care plans.
• Partner with patients, families, and healthcare providers to ensure seamless care delivery.
• Advocate for patients’ needs and preferences throughout the healthcare process.
• Collaborate with multidisciplinary teams to ensure timely testing, treatments, and follow-up.
• Balance clinical effectiveness with cost efficiency to optimize patient outcomes.
• Maintain knowledge of medical terminology, clinical workflows, medical record requirements, and regulatory standards.
  
  
  

Qualifications:

• Associate's Degree in Nursing (ASN) required; Bachelor of Nursing (BSN) preferred.
• Active RN license in Indiana or Nurse Licensure Compact (NLC).
• Graduate of a nationally accredited nursing program.
• 3-5 years of clinical experience preferred, ideally in relevant specialty areas.
• Basic Life Support (BLS) certification through the American Heart Association (AHA).
• Other advanced life support certifications may be required per department policies.
• Knowledge of medical terminology, electronic medical records, and healthcare regulations.
• Proficiency with computers and experience with word processing and database software.
