Job Summary/Overall Objectives

The CNC is a front-line care provider, coordinator, and leader within the healthcare team. The role involves applying the nursing process at the point of care to assess, diagnose, intervene, and evaluate the unit as a whole, aiming for systematic quality and safety improvements. A key aspect is enhancing interdisciplinary communication and coordination. The CNC helps patients and families navigate the healthcare system, advocates for them, and works within the unit, hospital, and system to improve care delivery, focusing on safe, evidence-based practices within complex healthcare systems.

Essential Job Functions

• Maintain professional knowledge through education, publications, networking, and professional societies.
• Ensure quality care by adhering to standards, measuring outcomes, and complying with regulations.
• Protect patients and staff by following infection control and medication protocols.
• Document patient care accurately.
• Communicate actions and needs among nursing teams.
• Maintain confidentiality of patient information.
• Establish rapport with patients and families to identify care needs.
• Create a compassionate environment with emotional and psychological support.
• Promote patient independence through education and goal setting.

Qualifications

• Graduate of an accredited Nursing Program.
• Bachelor’s Degree in Nursing (BSN); effective Jan 1, 2015, BSN required at hire or within four years.
• Minimum of one year experience in relevant nursing field.
• Strong leadership, organizational, communication, and legal knowledge skills.

Preferred Qualifications

• National certification in specialty area.
• Previous charge nurse experience.

Licenses and Certifications

• Current PA RN license.
• Current BLS certification from AHA.
• Act 33, 34, and 73 clearances with renewal.

Additional Details

• No direct supervisory responsibilities but may mentor staff.
• Scheduled based on operational needs; may include rotating shifts, standby, on-call.
• Travel between locations may be required.
