Job summary

Live Well Homecare Agency, LLC seeks an Ambulatory Float Pool Clinical Nurse to provide patient-centered care across outpatient settings, including specialty clinics and procedural areas. You will assess, plan, implement, and evaluate nursing care with a focus on safety, quality, and education.

Collaborate with multidisciplinary teams to develop individualized care plans, address social determinants of health, and ensure effective communication across the care continuum.

Qualifications

• 5+ years of RN experience (preferably in Critical Care, ED, or Infusion).
• BSN preferred; RN license required.
• Bachelor's Degree in Nursing and at least 2 years of experience preferred.
• Strong critical thinking, communication, and interpersonal skills.

Responsibilities

• Deliver high-quality clinical care, including patient assessment and education.
