Job Overview
Chicago Family Health Center (CFHC) is a mission-driven, community-based healthcare provider committed to delivering high-quality, culturally competent care to underserved communities across Chicago. We are currently seeking a Clinical Nurse with maternal health experience to join our Women’s Health team and make a meaningful impact on the lives of expectant and new mothers.

The Clinical Nurse will work under the supervision of the Director of Women’s Health and be an essential part of a multidisciplinary team dedicated to providing exceptional care to prenatal and postnatal patients. The ideal candidate is compassionate, organized, and experienced in women’s health, obstetrics, or maternal care.

Responsibilities:

• Develop and manage individualized care plans for pregnant patients, including comprehensive risk assessments.
• Serve as a care coordinator across a multidisciplinary team: Providers, Medical Assistants, Perinatal Family Coordinators, Referral Coordinators, and Maternal-Fetal Medicine specialists.
• Facilitate referrals to internal and external services: Dental, Nutrition, Behavioral Health, WIC, Healthy Start, etc.
• Monitor appointment adherence and patient compliance; conduct follow-ups to ensure continuity of care.
• Manage patients with coexisting medical conditions and follow up on abnormal test results.
• Act as a liaison between CFHC and Labor & Delivery departments at local hospitals.
• Educate and prepare patients for delivery: hospital tours, prenatal education, birth plans.
• Support postpartum care with services like newborn care education, breastfeeding support, and family planning.

Requirements

• Bachelor’s Degree in Nursing (BSN) required; Master’s Degree preferred.
• Graduate of an accredited nursing program.
• Current RN license in the State of Illinois.
• 2–5 years of nursing experience preferred; experience in maternal health, OB/GYN, or women’s health highly preferred.
• Case management experience with obstetrical patients is a plus.
• Bilingual ability is a strong advantage.
• Current CPR/BLS certification required.
