Job summary

Horizon Home Care & Hospice in Kenosha seeks a Clinical Manager to lead the day-to-day operations of the local home care team and branch. You will oversee RNs, PT, OT, ST and home health aides, ensuring high-quality patient care and survey readiness.

Responsibilities include optimizing reimbursement processes, coordinating withAgency leadership, and driving service opportunities while maintaining compliance with Medicare, Medicaid and managed care requirements.

Qualifications

• BSN from an accredited school of nursing.
• Current WI RN license is required.
• 5 years of nursing experience, preferably including one year in home health.

Responsibilities

• Manage the day-to-day operations of the Kenosha home care team and Kenosha branch office.
• Oversees Clinical Coordinator, RNs, PT, OT, ST and home health aides.
• Ensure regulatory compliance and survey readiness.
• Coordinate with leadership to implement new service opportunities.
• Oversee reimbursement processes across Medicare, Medicaid, managed care and commercial insurance.

Key responsibilities for this integral role include:

• Manage the day-to-day operations of the Kenosha home care team and Kenosha branch office
• Oversees the work of Clinical Coordinator, RNs, PT, OT, ST and home health aide.
• Understand the rules and regulations of home care and survey readiness.
• Understand the skills of the RNs to be available to assist in treatment, teaching and back up for coverage.
• Manages complaints regarding clinical issues presented by internal and/or external customers in a timely manner with implementation of a plan of correction.
• Manages clinical practice of the home care team ensuring employee practice meets all required guidelines.
• Manages specialty service lines (i.e. phlebotomy, WOCN, Telehealth, etc.¦)
• Demonstrates knowledge of care and services provided by acting as a resource for clinical issues or concerns.
• Works collaboratively with Agency leadership to implement new service opportunities as identified
• Demonstrates working knowledge of all aspects of home care reimbursement including Medicare, Medicaid, Managed Care and Commercial Insurance through managing the process of patient payers and authorization.
