Matter of Care Home Health Services in New York, NY seeks a Clinical Manager – Integrated Care to lead hospital-to-home transitions and coordinate high-quality care as patients return home. The role partners with physicians, discharge teams, and home health staff to reduce readmissions and improve outcomes.

The position requires an active NY RN license, BSN preferred, and 3+ years of clinical nursing with home care experience.

Responsibilities

As a Clinical Manager – Integrated Care, you'll play a vital leadership role in ensuring participants receive seamless, high-quality care as they transition safely home.
